Release and Indemnity Agreement Form

print one copy. After printing, you and a witness must both sign and date.
If you are under 18, your parent must also sign and date.
Local State Contract 2011
Open this document, complete the highlighted fields. Use

your parent/guardian must sign and date. In addition, the document
must be notarized.
Judges' Resume

The 2011
Miss Fort Worth

Miss
Teen Fort Worth
pageants will be held on Saturday, November 20, 2010 at Trinity Valley School
in Fort Worth.
Prize Package
Miss Fort Worth will receive an exciting prize package including:
- Cash scholarship
- $1,000 in-kind scholarship to the University of Texas at Arlington
- Wardrobe allowance and gift certificate to Regalia
- Initial fitness consultation with Dr. Larkin Page
- Initial hair care consultation with Art Diaz Salon in Dallas
- Official crown and sash
- Appearance scheduling throughout her year of service
